Please fill out the registration forms clearly. Your address will be used to send you important information.  Forms are due by 12/12/2010. 

All registration forms go to your local leader and the local leader sends all forms with a cover sheet and one check to the Overall Rector Deacon Don. Outside of New Mexico registrations go to Deacon Don.
